Arthroscopic removal of OCD lesion from shoulder (please be patient as the video loads - it is about a 5 MB file)
Take note of the following steps that you will see in the video:
1. An arthroscopic shaver is used to remove some of the layers of the flap so that the flap is thinner, more pliable and easier to get out of the small hole that is made in the joint.
2. The flap is detached from the humeral head with a hook-like probe (palpation probe).
3. The flap may fall in the back part of the shoulder called the cul de sac and require pulling the flap back into a region of the shoulder where it can be easiliy grasped the a forceps instrument.
4. The flap is grasped with the a forceps instrument and rolled around the end of the instrument - like rolling a spaghetti noodle on a fork.
5. The flap is then pulled out of the joint with the instrument.
6. The OCD bed is inspected to make sure that there are no more loose edges of cartilage - if there are these are trimmed back
7. The front of the joint (region of the biceps tendon/ medial collateral ligament/subscapular ligament) is inspected to make sure that there are no loose fragments left.
